RTF serialization support for Avalonia.Controls.Documents. Enables reading and writing FlowDocument content in RTF format using a streaming tokenizer.

### Features
- Bookmarks round-trip, so links into the document navigate after loading and after saving. `{\*\bkmkstart name}` names the paragraph holding it (`Block.Bookmarks`) and is written around every named paragraph; a link to a bookmark reads from and writes as `HYPERLINK \l "name"` (`HYPERLINK "file" \l "name"` reads as `file#name`). It was written as `HYPERLINK "#name"`, which Word treats as a file name. A markdown heading that a link targets by its slug is written as a bookmark under that slug.

### Fixes
- A markdown heading or code block (or any block kind registered for a `Paragraph` subclass) is written as a paragraph. The writer recognised only the built-in paragraph kind and dropped these blocks with their text.
